Submitting an Insurance Claim for Accident or Injury
- Complete Ontario Soccer Accident Claim Forms (5 pages).
- Submit completed forms to club for validation before sending to insurance provider (HUB International).
- Send forms and any supporting documentation (receipts, etc.) to [email protected] within 30 days of the injury.
- Contact [email protected] with any questions about claims or coverage.

Rowan’s Law
The Toronto Central Soccer Club complies with Rowan’s Law (Concussion Safety), 2018. We currently leverage resources made available to our Club as a member of the Toronto Soccer Association (TSA), including the Ontario Soccer Code of Conduct and the Removal and Return documents within the TSA “Concussion Protocol” as developed by Holland Bloorview Concussion Centre.
